PENDAHULUAN


Banyaknya kecelakaan yang terjadi, baik kecelakan kerja maupun kecelakaan transportasi, Human error seringkali dinyatakan sebagai faktor utama penyebab terjadinya kecelakaan tersebut. Bagi masyarakat awam, berita-berita tentang kecelakaan kerja/ transportasi dengan human error sebagai penyebabnya sering diartikan sebagai kesalahan manusia operator sistem (atau supir, masinis, pilot, dsb).

Persepsi ini sebenarnya kurang tepat, mengingat banyak faktor dan aspek lain yang dapat secara langsung maupun tidak mendorong seorang operator melakukan tindakan yang tidak tepat. Pentingnya pemahaman dan kajian yang mendalam atas human error di bidang transportasi dapat dipahami bila menilik besarnya kontribusi human error atas berbagai kejadian kecelakaan. 

Park (1997) Di dunia militer, 40 – 60% aspek manusia berkontribusi pada kesalahan sistem persenjataan dan pelayaran.  Angka ini sekitar 60% untuk penerbangan militer. Di dunia penerbangan pada umumnya, misalnya, sebuah estimasi (Shappell dan Wiegmann, 1999) menyebutkan bahwa sekitar 70 – 80% penyebab kecelakaan adalah human error.  Untuk sistem-sistem pada umumnya, 20 – 80% kegagalan terkait dengan human error.  Sisanya adalah karena keausan peralatan/ mesin, aspek lingkungan, dll.

Jumlah  kecelakaan yang cenderung tinggi dan besarnya dampak yang ditimbulkan menyebabkan perlunya pemahaman tentang sumber terjadinya kecelakaan kerja/ transportasi, terutama yang berkaitan dengan aspek human error.


TUJUAN :

  • Analisis dari investigasi diharapkan mengarah kepada sumber kecelakaan yang lebih akurat,
  • Dapat menetapkan strategi intervensi yang tepat dalam usaha meminimasi terjadinya kecelakaan atau meminimasi dampak yang terjadi akibat kecelakaan.
